Bacterial translocation from the gut to the mesenteric lymph nodes is a recognized complication of total parenteral nutrition (TPN). In a prospective, randomized study in an animal model, we assessed the effect of early bowel training on the incidence of bacterial translocation. Twenty-four rats received either TPN or somatostatin and an intestinal bypass for 10 days. Four rats served as controls. On the day of surgery all rats received the same TPN regimen, but only half of the animals were randomly selected to receive early bowel training, i.e., daily oral administration of 1 ml. dl-1 water for 6 days. Five days after surgery the rats were killed, and the incidence of translocation was determined by culture of mesenteric lymph nodes, mesenteric lymph nodes, liver, and spleen. Bacterial translocation was demonstrated by aerobic and anaerobic cultures in 2/6 animals on the day of surgery, 3/6 on the 7th day, and in 1/10 in the controls. No significant difference was found between the two treatment groups (6/12 vs. 6/12). In this experimental model, a 6-day training period was not sufficient to prevent bacterial translocation.
